Follow these steps to complete the Texas license process:
NOTE: Leave the bottom of the last page blank. It will need to be filled out in the presence of a notary public.
Step 1 - Download a copy of the industrial alcohol permit (Form L-102) using the link provided at the top of the page. You can fill out this form electronically in your browser, or print and complete by hand.
Step 2: Section 1A, Check off "Industrial Permit"
Step 3: Section 1B, check the box indicating if this application is an "Original" or "Renewal". See renewal frequency notes at the top of this page. Provide existing license number if this is a renewal. For Section 2 -Select the entity that best describes who is completing this application.
Step 4: Section 3 through 6 - Enter contact information of the business applying for the industrial alcohol permit.
Step 5: Leave section 7, 8, and 9 blank. Go to Page 2.
Step 6: Section 1A, B, C, D. Enter the requested business information. For Section B, this is also sometimes referred to as your EIN or Tax ID number. For Section C, Enter the official name of your business as registered with your state.
Step 7: Section 2 - Complete the information disclosure request for each officer, partner, director, and manager who has some controlling interest in the business applying for the industrial permit. Employees with the word manager need not be included on this disclosure form. Print as many copies of this page as needed.
Step 8: Skip to section 10 in the middle of the 3rd page.
Step 9: Section 10 "For Industrial Permit", explain the nature of your business and what you will use Culinary Solvent for.
Step 10: Leave section 11, 12, and 13 blank. Skip to the bottom of the last page.
Step 11: Acknowledgement section at the bottom of the last page. Leave the fields blank as a Notary Public needs to witness you filling those out and stamp their seal on the bottom left.
Step 12: Once L-102 is signed by the notary, gather Form L-102 along with the $381 application fee. Checks made payable to: Texas Alcoholic Beverage Commission
Step 13: Mail completed application and payment to: Texas Alcoholic Beverage Commission, P.O. Box 13127, Austin, Texas 78711.
Step 14: Receive back your approved permit in the physical mail. Allow 7-14 days for processing. If it's has been more than two weeks and you have not yet received a response, inquiries can be made to (512) 206-3333 Monday-Friday from 8:00am to 5:00pm Central Time.
