Follow these steps to complete the Michigan license process:
Download a blank copy of Form LCC-150 license application (pdf link above), and fill out the requested information as follows:
NOTE: If you open this form with Adobe Reader or Adobe Acrobat the sections with license fees will automatically calculate for you.
Step 1: Start with Part 1 on the second page.
Step 2: Fill out your name and address, and if you are applying as a company, fill in your FEIN
Step 3: Check "Yes" to only questions 1 and 7.
Step 4: Skip to the third page. Check the box next to "Limited Alcohol Buyer". Note: The "Limited Alcohol Buyer" license does not have an inspection fee, so it is only $10. It should look like the image below:
Note: If your form does not auto-complete you will need to fill out the fee calculation box (shown above) and also the fee calculation box in Part 4 at the bottom of page 2.
Step 5: Go to page 4. If you are applying as an individual, fill out Part 5a, 5b, and 5c. If you are applying as a corporation or LLC, fill out Part 5a and 5 c, as well as form LCC-301. Link to download above, Instructions for form LCC-301 here.
Step 6: (Part 5a) Enter your name, address, and contact information. Then answer the two questions. If you checked "No" to the first question and "Yes" to the second question, you must submit a copy of the completed and endorsed "Livescan Fingerprint Background Request" with your application. You do not need to click the link. The instructions and request are on pages 6 and 7.
Step 7: (Part 5b) This section is only for those applying as individuals. Start by entering your DOB, SSN, and Drivers License Number. Then answer the two questions. If you are married enter your Spouse's name and DOB. Then answer the next 5 questions.
Step 8: (Part 5c) Print and sign your name. Then enter the date.
Step 9: Go to page 5. Fill out Part 6 if the contact person is the applicant. Fill out Part 7 if the contact person will be an attorney.
Step 10: Print your name and title. Then sign and date the document. (If you are required to submit the fingerprint scan proceed to pages 6 and 7 for instructions.)
Step 11: Once the application is complete, check page 1 and make sure you have all the documents you need. Then mail them with your fees to:

Wait to receive back your approved license via mail. Allow 30 days for processing. If it has been 31 business days and you have not yet received a response, inquiries can be made to 1-866-813-0011 Monday-Friday from 8:00am to 5:00pm Eastern Time.
If there is a problem with your application you will receive mail detailing the issue and suggested corrective measures.
Once you receive your license you will need to fill out one more form. The "Release of Alcoholic Beverages for Commercial Use" is an order form that needs to be approved by the state.
